Folate 5-MTHF is supplied in capsule form, with 300 capsules per container. Each capsule provides 400 µg of folic acid in the form of the glucosamine salt of (6S)-5-methyltetrahydrofolic acid, known as Quatrefolic®.

The capsule shell is made from hydroxypropylmethylcellulose, and the filling agent is microcrystalline cellulose. No allergens are explicitly declared on the label.
