Vitamin B-12 Ultra is a liquid supplement supplied in a 118 ml (4 fl oz) bottle, providing approximately 23–24 servings. Each serving is 1 teaspoon (5 mL).

The formula provides Vitamin C (as Ascorbic Acid) 120 mg, Thiamin (Vitamin B-1) 3 mg, Riboflavin (Vitamin B-2) 3.4 mg, Niacin (Vitamin B-3) 50 mg, Vitamin B-6 10 mg, Folate (as Folic Acid) 800 µg, Vitamin B-12 5 mg (5,000 µg) as Cyanocobalamin, Methylcobalamin (Co-enzyme Form) and Dibencozide (Co-enzyme Form), Biotin 300 µg, and Pantothenic Acid (Vitamin B-5) 50 mg. Other ingredients are de-ionized water, vegetable glycerin, xylitol, L-malic acid, natural flavors, organic stevia leaf extract (enzyme-modified steviol glycosides), potassium sorbate (as preservative), grapefruit fiber and cinnamon bark oil.
